New Account Inquiry Form

This type of account enables you to purchase products at United Products with cash, check, money order or credit card when the order is placed. An account is setup as a "cash account" but enables you to track purchases on your account. If you are inquiring about setting up a new credit account but would like to purchase today, please fill out both the  New Account Inquiry Form for immediate purchase and a Credit Application form to process for a new credit account.

Credit Application

This form is to apply for a credit account at United Products. Once opened, you will have a credit line to use for purchases and be assigned a sales representative. Fill out the application and fax or mail it back to us.
